ADVICE NURSE
Location: West Salem Clinic | 1233 Edgewater St.
NW, Salem OR 97304
Job Status: Full-time, Monday -Friday
YOUR ROLE:
As our Advice Nurse, you will dispense telephone advice to patients seeking help for their health concerns.
Using your keen clinical skills, you will assess their health concerns, provide education, and advise them of next steps.
You will collaborate with each patient’s assigned provider to facilitate patient care while recognizing each patient as the central focus while offering trauma-informed and compassionate support.
SPECIFIC DUITIES:
Serve as the telephone advice nurse and follow established policies.
The Advice Nurse schedules appointments as appropriate and communicates effectively in a professional manner with clients, staff, and providers.
Receive incoming phone calls treating each patient in a courteous and respectful manner using a while assuring that the patient understands the information given.
The Advice Nurse maintains accurate and complete updated records in patient charts.
Participate in the ongoing management of patients with chronic diseases, as designated by and in consultation with the Primary Care Provider (PCP), via communications with patients regarding issues concerning management of their chronic disease, coordination of treatment plans/changes, as well as coordination of services.
Includes follow-up with the patient when indicated.
Employ strategies that promote the health and wellness of individuals/families and populations.
This includes ways to maximize wellness, prevent illness, and manage acute and chronic disease and disabilities to impact positive health outcomes.
Communicate progress of referrals to clients, as well as tests results following PCP provider test action.
Thoroughly documents all communication in the EHR.
Authorize referral to Convenient Care or ER based on phone triage.
QUALIFICATIONS:
Current Oregon RN/LPN license.
CPR certified for infant, child, and adult levels.
Clinic experience is preferred.
