New Foundations LLC is an operator of Mental health Residential Treatment homes providing services to 5 individuals living in a home-like setting.
We are looking for someone to join our team that is outgoing and motivated that will provide services to help better our program in serving the residents living in our home with SPMI as well as other co-occurring diagnosis.
Applicants can expect the following as to be part of their job description:
* Conducts assessments and screenings of residents upon entry to program.
Assessments include but are not limited to mental health (yearly), ISP (yearly, quarterly, monthly)
* Prepares and maintains all required treatment records and reports.
* Counsels clients in individual and group counseling sessions as needed.
* Collects information about clients through interviews, observations, and collateral information.
* Assists with training of and support to direct care staff.
* Acts as client advocate to coordinate required services or to resolve emergency problems in crisis situations.
* Develops and implements evidence-based, client-centered treatment plans that address mental health and substance use.
* Documents relevant treatment reviews, progress notes, status reports, coordination of services, and discharge summaries.
* Participates in monthly administrative meeting.
Qualifications-
* Requires QMHP with a graduate degree in psychology, social work, or a behavioral science field from an accredited college or university with proof of license/certification.
Preferably with 3 years' experience in this field.
* Experience must demonstrate the ability to conduct an assessment, including identifying precipitating events, gathering histories of mental and physical health, substance use, past mental health services.
Conducting a mental status examination; complete a DSM diagnosis; write and supervise the implementation of a service/treatment plan; and provide individual, or group therapy within the scope of their training.
Knowledge, Skills and Abilities
* Excellent interpersonal communication skills and strong computer and time management skills are required to work effectively.
* An intermediate understanding of Oregon Administrative Rules (OARs) is preferred.
